RECONNECTION FEE 

  • P200 (If reconnected within ten (10) days from the date of disconnection)
  • P400 (If reconnected for more than ten (10) days from the date of disconnection)

CALIBRATION FEE 

(By size of water meter, and to be charged to the requesting concessionaire)

A.  REQUIREMENTS FOR NEW SERVICE CONNECTION

      Basic Requirements

  • Lot Owner
  1. Valid ID (must be government issued i.e. SSS, Voter’s ID, GSIS, Postal, TIN, Passport)
  2. Barangay Clearance (Original)
  3. Land Title/Tax Declaration or Deed of Sale (Notarized)
  • Related to Lot Owner
  1. Valid ID (must be government issued i.e. SSS, Voter’s ID, GSIS, Postal, TIN, Passport)
  2. Barangay Clearance (Original)
  3. Land Title/Tax Declaration w/ Authorization from Lot Owner & ID
  4. MOWAD Waiver (Notarized)

     If the lot owner is deceased:

  • Barangay Certification (relation to lot owner) instead of Barangay Clearance
  • Tenant/Lessee
  1. Valid ID (must be government issued i.e. SSS, Voter’s ID, GSIS, Postal, TIN, Passport)
  2. Barangay Clearance (Original)
  3. Lease Agreement or Contract

    -OR-

  1. Authorization Letter from Lessor w/ Valid ID
  2. Guarantee Fund (P500.00) (if the lessor has no proof of ownership)
  3. MOWAD Waiver (Notarized)

 

B. REQUEST FOR RELOCATION FEE

  1. Valid I.D. (must be government issued i.e. SSS, Voter’s ID, GSIS, Postal, TIN, Passport)
  2. Barangay Clearance of person applying for change name
  3. Authorization letter w/ valid I.D. from the registered owner of the account

a. In case the registered owner is deceased:

  1. Baranggay Certification instead of Brgy. Clearance
  2. Deed of Sale or
  3. Land Title or
  4. Tax Declaration or
  5. Barangay Certification of Relationship to Lot Owner

b. In absence of requirement 3, payment of Guarantee Fund ( P 500.00) is required.

C. REQUEST FOR RECONNECTION

Submit Request for Reconnection signed by the registered owner of account and one (1) xerox copy of any valid I. D.

D. REQUEST FOR TEMPORARY DISCONNECTION

Submit Request for Temporary Disconnection signed by the registered owner of account and one (1) xerox copy of any valid I.D.

E. REQUEST FOR RELOCATION

  1. To be relocated at same address:

Submit Request for Relocation signed by registered owner of account and one (1) Xerox copy of any valid I.D.
